OBJECTIVES:

     Upon successful completion of this course, students must be able to:

 

•             Calculate the delinquency rate for facility medical records by applying the correct formula (BL3)

•             Report various uses and functions of the health record (BL2)

•             Articulate the distinction between an incomplete medical record and a delinquent medical record and their effect on patient care, facility reimbursement and compliance with regulatory standards (BL2)

•             Explain compliance strategies to ensure adherence to legal and regulatory standards in health information management (BL2)

•             Recognize deficiencies in facility policies for compliance with internal and external standards (BL2).

 

Tasks: Given a report with the incomplete medical record count for a given year, calculate delinquency rates for each month for the AHIMA Medical Center. In a report that will be presented to administration, indicate the number of deficiencies/incomplete records for each month by physician, in preparation for the upcoming Joint Commission survey. Include the delinquency rate for each month. Presentation must include the distinction between an incomplete/deficient record and a delinquent record. This report will be presented to members of the Board of Trustees for review at the next board meeting.  A draft of this assignment is due before the submission of the completed, corrected project.

 

Part I:  Given a table with the aggregate number of incomplete/deficient medical records each month for the past year, calculate delinquency rates for each month, using the appropriate formula. The name of your hospital is, AHIMA Medical Center and should appear as part of your heading.  The title of your table/spreadsheet should be, Monthly Delinquency Rate. Your assignment includes the following:

 

 

 

 

•             Number of incomplete/deficient charts by month (data provided); You must break this number down according to the physicians whose records are deficient for each month (you allocate the incomplete medical records to attending physicians in your facility in table format to recognize trends);

•             Number of discharges per month (data provided);

•             Monthly delinquency rate (calculation required using the formula below);

•             Indicate as part of your report, whether the delinquency rate is increasing, decreasing or remaining around the same amount each month. This should be compiled and presented in table format (either excel or word) (This is the same table in which you would include and allocate the deficiencies/delinquencies to the attending physicians).

 

The formula to calculate the delinquency rate is:

 

                Number of total deficient records for the month

                      _________________________________________  x100

                Number of total discharges for the previous month

 

 

Example:             30 (Total incomplete records today)

                         600 (Total number of discharges from last month = .05

Multiply .05 x 100 = 5% which is the delinquency rate, and is very good.

 

Part II: Review the current policy for your organization on procedures for tracking and completing medical record deficiencies (attached).  Based on research, lectures and assigned readings, explain what should be added to the policy for increased compliance by physicians. This should also contain information on distinction between an incomplete/deficient record and a delinquent record, the effect delinquent records can have on the quality of patient care and the financial impact of delinquencies for the facility and any trends that can be seen in the illustration of the monthly delinquency rates. 

 

You must compile your findings in a report that must be 2-3 pages in length, double-spaced and typed in Times New Roman, size 12 font, using an APA formatted reference page to list any sources used for this assignment.The completed table of delinquencies must be included with your report as an additional page.
