- Fully define the presenting problem(s) and concerns of your client.
- Identify the symptoms and indications of the problem. (How is it manifesting itself? What are the consequences? How severe are the client problems? What meaning do clients ascribe to the problem?)
- Identify the DSM-5 diagnosis of the client.
- Describe strength and growth/problem areas (i.e., areas to be addressed in Tx plan) of the client.
- Identify and give a rationale for at least two therapeutic approaches that could be used with this client. (The therapeutic approaches you select should be based on content from the Prochaska and Norcross text and the material covered in SW 728; each therapeutic approach should be distinctly different— i.e., represent content from two different chapters in the text.)
- Identify at least one overall treatment goal and three corresponding objectives that are specific, measurable, action oriented, realistic, and timely.
- Include specific interventions for each objective; objectives must include targeting behaviors/symptoms, must be measurable, and must include frequency and target dates.
- Give thought to the uniqueness, preferences, and values of the client in developing goals.
- Treatment objectives and interventions should demonstrate use of the two therapeutic approaches identified and discussed above.
- Discuss at least two personal and/or professional ethical issues you have faced (or anticipate facing, depending on your work with the client)
