Creating Reports and Forms

GETTING STARTED

  • Open the file SC_AC19_EOM4-2_FirstLastName_1.accdb, available for download from the SAM website.
  • Save the file as SC_AC19_EOM4-2_FirstLastName_2.accdb by changing the “1” to a “2”.
    • If you do not see the .accdb file extension in the Save As dialog box, do not type it. The program will add the file extension for you automatically.
  • Open the _GradingInfoTable table and ensure that your first and last name is displayed as the first record in the table. If the table does not contain your name, delete the file and download a new copy from the SAM website.
  • PROJECT STEPS
  1. Rural Health Associates is a healthcare company that manages rural health clinics in underserved areas of the country. Rural Health Associates provides low-cost health care based on income level. You work in the Patient Services Department as an information specialist.

Clinic managers have asked you to modify some forms and to create a new form. Open the ProfessionalStaff Update Form in Layout View and bold the Position control. Save and close the form.
  2. Upper management does not need all the detail shown in the Family Income Report. Open the Family Income Report in Layout View and then create a summary report. Save and close the report.
  3. Clinic managers need to easily identify patients with higher incomes. Open the IncomeLevels by State report in Layout View. Average the values in the StateMinInc column. If a value in the TaxIncome column is greater than $35,000, the value should display in a bold, green (last row, column 6 of Standard Color palette) font. Preview the report to check that the values in the subtotal and total controls display completely. If necessary, expand the size of the controls. Confirm that your report matches Figure 1, then save and close the report.

  1. Staff have asked for a form that identifies clinics and locations. Using the Form Wizard, create a form for the Clinics table. Include the ClinicID, ClinicName, City, and State fields on the form. Use a columnar layout for the form and use Clinic Location Form as the name of the form. Close the form.
  2. The Kalispell Clinic needs some specific information on its patients. Open the Family Update Form in Form View. Create an advanced filter that sorts the LastName field in ascending order and restricts retrieval to only those records where the State field value equals "MT." Toggle the filter to confirm that it is working. (Hint: There should only be six records included in the form.) Save and close the form.
  3. Open the Clinic Update Form in Layout View, and then add the current date to the form using the format day-month-year (e.g., 11-Mar-20). Do not include the time. Save and close the form.

Save and close any open objects in your database. Compact and repair your database, close it, and then exit Access. Follow the directions on the SAM website to submit your completed project.
