This course project has three (3) key parts: Note: This is a comprehensive project, and you should start working on it from week 1. Use the textbook and all supplemental reading resources provided in this course. This project's main focus will be integrating evidence-based literature, critical thinking and synthesis, and problem-solving. Part I (Due Day 7 of Week 7) Your Final Project paper will include the following (10–12 pages). Provide headings that separate and define the different sections of the assignment. Executive Summary (1 page) Topic Target audience Scope Key stakeholders Time frame Operational and budgetary resources Outcomes and goals Evaluate and measure (during and post-implementation) Population Health Management Program Proposal (Page 1) (Mission, Vision, and Values) Data Analysis Summary(1-2 pages) Summarize in paragraph form key relevant information from your Data Analysis (you can also submit the worksheets as Appendixes supplement) Strengths, Weaknesses, Opportunities, and Threats (SWOT) Analysis (1 page) Operational Resources Requirements Needed to Support this Program(1 page) Provide written and visual presentation of SWOT as table or matrix Community Partnerships (1 page) Social Change and Impact on the Targeted Community (1 page) Program Evaluation (1-2 pages) Explain in one (1) page how you would plan to measure social change. Conclusion (1 page) References: Please include at least 15 current high-quality references (no more than four years old) and at least 10 peer-reviewed articles pulled from the GWU library. Start collecting and building your list of references early on in the course. Use the GW library to locate peer-reviewed articles. Part II (Due Day 7 of Week 7) Prepare a professional 10-12 content slides PowerPoint Presentation (PPT) delivered to your health system's Board of Directors. The slide count does not include title, references, and additional supplementation slides. There is no need to present the ppt. Virtually but you need to submit to the assignment link. The ppt. should include detailed speaker notes in each content discussion slide. The minimum required word count for speaker notes is 150 words per slide. Part III (Due Day 1 of Week 8) On Day 1 of week eight, submit your final project paper in the designated discussion question thread under Week Eight (8). Your classmates will read your project and offer you feedback, suggestions, observations, and ideas for improvement. You are required to review at least two (2) projects and provide meaningful and constructive feedback that helps your classmates obtain a second opinion on this project. The peer-revised feedback should be substantive, listing the stronger elements presented and the opportunities for improvement to be considered in the project.
