Your patient is an 18-year-old women's soccer athlete who is in her senior year of high school. She reports that she has been clinically diagnosed with an ACL tear in her right knee. She reports no previous history of knee injuries. She reports being limited in the ability to bend her knee or fully extend due to the amount of swelling in her right knee. The patient also reports difficulty with ascending and descending stairs and activities of daily living (i.e sitting down to go to the bathroom, getting comfortable in bed, getting in and out of the car). Upon evaluation, the physician assistant (PA) finds strength deficits during manual muscle testing and diffused knee pain. The patient reports being upset because she cannot "even kick a ball or dribble on the sidelines of soccer practice". The patient confirms that she has been removed from all participation in soccer-related drills but is still able to attend school with the use of crutches to ambulate. The patient expresses that her teammates, friends and family have been supportive but she is stressed that she "let down her coaching staff" just before the playoffs were about to begin. Additionally, your patient is concerned that she will lose her athletic scholarship to Boston College because she will not be able to train throughout the remainder of the academic year. Your patient appears very driven to create a concrete schedule of her recovery timeline and appears well educated in the nature of her injury. Patient 2: You are a mental health counselor at Dunder Mifflin University who is meeting with a 22-year-old trans male swimmer who has been experiencing panic attacks in large social settings. The patient reports chest pains, racing heart, dizziness, and sweating while at parties when he is engaging in drinking. He has a history of psychological counseling and has been clinically diagnosed with generalized anxiety, and panic attacks. The patient reports that he has experienced these for approximately 10 years. The patient has utilized a variety of coping strategies to manage his stress and anxiety (such as therapeutic yoga, diaphragmatic breathing, and writing) but has not been incorporating these into his current management strategies. Additionally, while swimming has been a coping strategy and he is not currently restricted from participation he is worried that his symptoms may start to creep into his performance. Your patient reports that his parents have not been very supportive through his mental health journey and constantly communicate in a way that emphasizes stigmas surrounding mental health considerations. Furthermore, your patient reports that he only speaks to his girlfriend about what he has been experiencing but has yet to discuss this with his swimming coach, teammates, or peers.
