Purpose:  The purpose of this assignment is to recognize and develop attributes of a professional nurse needed in the care of a client with chronic disease.  You will learn about challenges faced by clients suffering chronic disease and develop skills in leadership, collaboration, ethics and social justice, helping relationships, and coping with stress, which will benefit you in your future nursing career.  

 

Objectives (Skills): The purpose of this assignment is to help you reflect on these objectives which are essential for your success in this course, in the nursing program, and in your professional life beyond school:

    

  1. Discuss professional values necessary to work with clients experiencing chronic disease or end of life and reflect on your strengths and challenges in these areas (1G).
  2. Reflect on ways to maintain your own physical, psychological and spiritual health to allow you to deliver sustained quality care (1J).
  3. Describe the roles of nurses and other professionals in caring for clients experiencing chronic disease or end of life (2E).
  4. Discuss leadership qualities that enable nurses to collaborate with other professionals to ensure quality care for clients experiencing chronic disease or end of life (2A, 2E).
  5. Discuss ways you could advocate for social justice for vulnerable clients experiencing chronic disease or end of life (7O).
  6. Consider your ability to establish helping relationships, communicate professionally and identify strategies to promote your own professional growth.

  1. Set up an interview with someone experiencing chronic disease or end of life care who needs frequent contact with health care professionals. The interview may be done in person or on the phone (practice physical distancing).
  2. Ask the client about their health care needs, their health maintenance, accommodations in their living environment, and any other factors that influence their living with chronic disease.
  3. Consider the skills and attributes required of professionals who work with the client.
  4. Reflect on your professional skills and attributes and identify opportunities for growth.
  5. Complete the journal using these headings:

 

A.  Experience 

 

a. Describe the client including any health conditions or disabilities.

b. Describe the client’s health maintenance.  Do they require assistance with ADLs and IADLS?  Who provides this assistance?  What medications and therapies do they require?

c. Discuss the client’s living environment noting any accommodations they need.

d. Discuss the client’s self-maintenance and some of the factors that influence the manner in which they live with their chronic disease.  These could include but are not limited to:  support systems, finances/insurance, cultural/ethnic and spiritual/religious factors.  

 

B.  Professional Roles & Leadership Skill that Facilitate Interprofessional Collaboration

                 

a. Identify the health care providers and agencies (clinics, hospitals, dialysis centers etc.) that provide health care for the client.

b. Discuss the roles of several professionals, including nurses, in providing this client’s care.  

c. Identify ways professional collaboration improves the safety or health outcomes of this client. 

d. Research and discuss at least three important leadership skills needed to facilitate the promotion of inter-collaborative work and tell why you believe these skills are important in promoting quality health outcomes for clients experiencing chronic disease or end of life.

 

Note: Inter-collaborative is amongst varied professionals and not just NAs and nursing staff.

 

       C.  Ethics and Social Justice

 

a. Reflect on the professional nursing ethics of autonomy, nonmaleficence, beneficence, fidelity and justice.  Discuss the importance of these values in provision of care to clients    experiencing chronic disease and/or end of life.

       b. Give an example of a way you could demonstrate autonomy, nonmaleficence,          beneficence and fidelity while providing care for clients in situations similar to that of your client. 

c. Discuss a potential health care disparity (inequality) your client and others in similar

     situations might face.  Give a detailed example of how you could promote social justice   with this issue.

d. Thoroughly cover the topic of social justice and how this might apply to your client. 

 

D.  Helping Relationships

 

a. Helping relationships are specific, therapeutic relationships established between nurses  

    and clients.         

b. Identify the qualities of a helping relationship.  Discuss ways you can promote effective communication and establish a helping relationship with clients experiencing chronic disease or end of life. 

c. Identify types of clients you find it difficult to work with and discuss challenges you have in developing therapeutic helping relationships with these clients. 

d. Discuss strategies you can use to grow in this area.

 

Note:  A helping relationship is defined in the required reading.

 

E.  Physical, Psychological and Spiritual Stress:

 

        a. Identify some of the physical, psychological and spiritual stressors you experienced or

believe you would experience if you were working with clients like the one you interviewed.  What did (or would) you find difficult or challenging?

        b. Research and discuss ways you could address physical, psychological and spiritual   

      stress to ensure you were able to deal with it and deliver quality care at all times. Be

            specific.
