You will formulate and communicate a feasible implementation plan and offer a careful evaluation plan for your community health program. Your implementation and evaluation plan should be based on your change and program theories.

The purpose of this assignment is to demonstrate your ability to justify the approach you intend to take to your program and its likelihood of achieving a positive health effect.  

Instructions: 

Format?:    Papers should be 3-5 pages long, double-spaced, 12-font and 1” margins all around. APA format should be followed throughout, including all cited references.  References are not included in the 5-page limit, and should include all data sources. Quality of writing and APA is worth 5pts.

Program theory Logic Model (5pts) 

Construct a visual conceptual model of your program with assumptions, inputs, activities, and outputs that are clearly related to your intended program goals/outcomes. Provide a written annotation to explain and justify its contents. 

 

Timeline, budget, and budget justification (5pts)

Provide a high-level 24-month timeline that represents your work plan. Create a budget outlining the resources needed to complete your program activities. Budgets should adhere to the parameters outlined in the RFP, so please review carefully.  Write a budget justification explaining the purpose of each expense and how you calculated the estimated cost. Identify risks to your suggested budget and describe how these risks will be managed. Include evaluation costs in your budget. ?The timeline and budget worksheet do not count toward the  3-5 page limit and can be attached as an appendix to the paper. Templates for timeline and budget will be provided. 

 

Process Evaluation Plan  (5pts)

Describe how you will evaluate the process of implementing your program. Identify specific process measures from your program plan and describe how you will evaluate those measures (method, how often). Make sure you include assessments of whether your target population is being reached, whether the program is implemented as planned, and how you will remedy under-performance of your program plan. 

 

Effects Evaluation Plan (5pts)

Describe how you will evaluate the effectiveness of your health program. Identify specific health outcome measures as well as intermediate behavioral or environmental change outcomes of interest. Describe how you will evaluate those measures (method, how often) and how you will evaluate upstream influences on these outcomes. Make sure to include an assessment of whether you can or can’t evaluate long-term impact (like reduced mortality). 

 

Peer Review (5pts): ?Provide a formal peer review of one classmate’s needs assessment using a  template provided by the instructor. I will assign you a peer to swap with and the template.
