You were hired as the Director of Health Information for a very busy

nursing home/rehabilitation center. They currently use a manual tracking system for Release of Information Requests, but the CEO wants to modernize the system and has asked you to develop an Excel spreadsheet for this purpose. He wants you to enter the most recent requests from the manual log and generate the following reports:
1)   Number of invalid requests;
2)   Number of requests from attorneys;
3)   Number of requests not responded to within the necessary time frame;
4)   Number of requests still pending;
5)   Total fees paid from 3/31/20 - 4/24/20.
This must be done using appropriate formulas in Excel. The spreadsheet must also contain all necessary information for compliance with federal and state laws and facility policies. The headings in the manual log may not necessarily be all that are necessary, or in a way where you can calculate fees paid. You must determine this based on your knowledge of the legal requirements and use of Excel. The legal references must be cited on the spreadsheet. The attached contains the information from the manual log that must be entered.
Make sure your form is formatted professionally-looking and that when it prints, it looks good (not all over the place).
