Case 2-9. E-Discovery Madison Medical Center has implemented an EHR, and all patient information since October 1, 2020, has been stored in the EHR. A subpoena has been received that request the following information.

• Entire record for Mary Ellen Smith's hospitalization of 12/1/2020-12/12/2020. 
• The names of all individuals who have accessed her health record. 
• The specific information that each of the individuals viewed. 
• The clinical practice guidelines that were used when providing care to the patient during the hospitalization.
