the population so that the reader knows and can see in their mind’s eye what your population is

Assess the needs of the population you have selected – 10%

Look at surveys? Look at the department of health? Tell us in this section what you did to learn

what health concerns your population had.

Describe how will you implement the interventions 15% - will you have meetings?

Flyers? Host classes? Attend a community board meeting and make a presentation?

How much will this cost? Any barriers?

Evaluation of results

How will you measure the results of your intervention? 15% - Will you do a survey? Will

you do medical tests? Will you measure deaths? Illnesses? How will you know how good your

intervention is?  Be concrete – how will you know?
