Required Submissions

  1. A typed cover letter indicating the following:
    1. The field placement being challenged and the program of which it is a part
    2. Student’s name, student number, address, telephone number(s) and e-mail address(s)
    3. The name and/or title of the service provider(s) being cited for the eligible experience(s)
    4. The name, title and credentials of the supervisor(s) of the eligible experience(s)
    5. Contact information (telephone number and/or e-mail address) for the supervisor(s) and/or the current director of the agency/service provider if the supervisor is no longer with the cited service provider
    6. The dates of each supervised experience
    7. The total hours of each supervised experience

  1. A typed letter/document regarding each supervised experience that includes:
    1. a detailed description of the job and/or volunteer functions and responsibilities being assessed for PLAR
    2. the number of hours being used for the challenge
    3. a statement that the supervisor(s) has read and confirms the information contained within the “competency paper”
    4. the signature of the supervisor(s) and/or the current director for the service provider indicating that they support the information contained within the letter/document.  The letter/document must indicate the signer’s current job title/function and be on letterhead.

  1. A typed competency paper that describes in detail how the student met each of the “course specific” learning outcomes for the field placement course.  The “course specific” learning outcomes are found in the left column on the second page of the relevant course outline.  Students should also refer to the “enabling objectives” for each outcome to assist them in preparing the competency paper.  The enabling objectives for each applicable learning outcome can be found in the accompanying evaluation form for the appropriate field placement.  Any enabling objective that was not met, or that the student believes is not applicable, should be addressed in the competency paper. The enabling objectives should be described in detail with an example from the client agency work.

For a successful challenge of the SSW field placements, all submission requirements and all learning outcomes must be clearly addressed.  The competency paper should be double-spaced and use a 12 point font.  Each page must be numbered. The average number of pages is 10 to 15, of clearly documented enabling objectives, using standard English language. The paper should be proofed for spelling and grammatical errors. The paper must be signed by your prior supervisor.

The outcome of the SSW Field Placement challenges will be “pass” or “fail”.

Documentation must be completed and submitted to the PLAR office within 20 working days of registering for the PLAR challenge.
