HS 252 Co-Occurring Disorders. Case Study#2-

Jane has come by the free “drop-in” counseling clinic to get help. Her case was assigned to you.  Here is the client's information per your intake:

She is a 22-year-old single Caucasian woman who has been living with her boyfriend, John, 32 years old, Caucasian, for the last four years. They both have been heroin addicts for many years. Jane claims to have started using heroin at the age of 17. She discloses her family history as being dysfunctional and abusive. She discloses that her father was an alcoholic and abusive. Her parents divorced when she was 14 years old. Her father died in a car accident when Jane was 16. She has one sibling, an older sister 23 years old that is a kindergarten teacher. Her mother, currently 52 years old, has some medical issues and on disability and lives with the client's sister. Both her mother and her sister see Jane a few times a year when Jane reaches out to them asking for help mostly money. Recently they told her that they will no longer give her money but only food and clothing. 

Jane has dropped out of high school when she was 16 years old and worked in a local grocery market for one year. Around the age of 17, she started using heroin and living on the streets. She says" As far as I remember, I have always been an unhappy person. I remember I missed my dad so much even though he wasn't a good father. My mom hated me for that. My sister is the perfect child. I am such a disappointment, I guess" 

She met John on the streets and moved in with him four years ago. They have a room in a two-bedroom apartment with a few other roommates. She discloses that she is a sex worker in order to be able to pay her rent, her bills and take care of herself and John, as well as to be able to buy her drugs. She says "John Doesn't work but he is a great man. I have never been happier. He is very supportive and protective of me. He doesn't let anyone in the streets hurt me". She continues to say: "if we can’t score enough heroin, John gets us Valium or other similar pills to tide me over until we can get some heroin.” She tells you that alcohol and heroin help to calm her nerves and get her through the night. Finally, she discloses to you that she is pregnant. Her pregnancy test was positive but she doesn't know how far along she is. She says she has missed a few of her menstrual cycles. She discloses that both she and John want to keep the baby. She indicates that her pregnancy and her baby is the reason she is seeking help. 

 

You must develop a treatment plan for this case using the Themes to assess the client’s symptoms. Your Goals and interventions must include all criteria: Psychological/Psychiatric, Medical/Biological, Environmental/Social, and Legal/Ethical.

Required: -Minimum of 7 symptoms, 14 goals, two per symptoms, 14 interventions one for each goal. Refer to Canvas for the grading rubric.
