Assignment 1: Needs Assessment/Problem Definition 

15% of course grade / 30 points 

You will conduct a thorough assessment of the public health literature, relevant local data sources, and a community needs and assets, and Healthy People 2030 objectives in order to justify the need to address the health issue of your chosen RFP in your target community. Use as wide a variety of resources as you can find to provide a complete explanation of the problem and justification of the need for a program to address the problem.

The purpose of this assignment is to demonstrate your ability to utilize several different sources of information to construct a complete assessment of a community health issue.

Instructions: 

Format:  Papers should be 3-5 pages long, double-spaced, 12-font and 1” margins all around. APA format should be followed throughout, including all cited references.  References are not included in the 5-page limit, and should include all data sources. Quality of writing and APA is worth 5pts.

Literature Review of the health problem (5pts)

In this section, summarize the peer reviewed literature about the public health issue in the RFP you are responding to. Describe the health problem including the size and scope of the problem and any disparities in risk factors or outcomes that impacts your target community. Describe the Healthy People 2030 objectives related to this problem.

Community identification and assessment (10pts)

In this section, utilize several types of resources to describe your target community and how the public health issue impacts that community. Describe the community assets and needs as they relate to the public health issue and competing priorities in the community. Also describe identify the primary, secondary, and other key stakeholders and find resources to understand how those stakeholders may view the problem. Consider and describe how the public health problem relates to existing community priorities. Use local public health data, news articles, social media discussions, and any other evidence you can locate to create a complete picture. 

Logic Model of the Problem & Health Outcomes (5pts)

Using your findings, create a logic model of the problem to depict the social-ecological factors that influence health behaviors and contribute to the health problem described in your scenario. Propose 1-2 specific health outcome(s) of interest for your program.
