Make sure to include the HPI (History of Present Illness) and what YOU believe to be priority patient problem(s). Use your textbook and other resources to help you. You need to prioritize the main patient problem(s) on your concept map. 4. Please list both objective and subjective assessment findings that are pertinent to the patient problem(s). 5. Next, write the plan of care and the nursing interventions for this client. Use your textbook as a reference. You will want to include ALL interventions related to the client's outcomes/goals and you must include at least 2 or more independent and 2 collaborative interventions. 6. Complete the evaluation of the plan of care/nursing interventions. This information must relate to the patient outcomes/goals and the appropriateness of the nursing interventions. (Remember - you must also actually perform these nursing interventions with your patient in clinical!) Were the goals met, partially met or unmet? Documentation of the patient's response to interventions must be specific, thorough and concise.
