Samuel, fourth grade

Samuel is a friendly student who makes friends easily and has a good relationship with staff.

He tends to get off task during reading assignments and has difficulty with problem solving skills. He regularly turns in his homework late or not at all for his reading assignments. He will often give up if his reading assignment if he has to write long paragraphs. When given multiple reminders, he will turn in his work on time. He forgets to write in his planner what his homework is and his backpack is filled with old papers.

He understands content when it is read to him but has difficulty reading material on his grade level. His phonemic awareness skills are on a first grade level and his phonics skills are on a second grade level. His oral vocabulary is a sixth grade level.

The Key Math Assessment shows Samuel has not memorized his single digit multiplication tables and division tables. He uses his fingers for addition and often confuses himself. When presented with orally read word problems, he can develop a strategy and solve an equation.
