Continue with your evaluation from last assignment using the same issue/problem. Using the methods learned in this class, your data analysis, and your research, develop and submit your response. Insert graphics into the sections where requested and/or needed (as opposed to adding an appendix). I recommend exceeding the page length requirements for best results. 5 – 7 pages is requested. Please exceed that for best results, especially since your graphics should be a large part of it; deep inclusion of graphics is encouraged. This paper relies heavily on expert opinion. Theme/Title and Background: Provide a Title that focuses on the problem and not one that is prescriptive of solutions. In terms of Background, write about any relevant information that might be important in order to understand the importance, impact or extent of the problem and how the problem relates to any organizational strategies or goals. Analyze data trends for this measure over time.
Current State: Use graphical display(s) of data to illustrate the gap between current quality performance and a desired standard. Show "what/where is", as well as "what/where is not,” the problem (for example, you could break down a departmental no-show rate by division, showing the department with the highest no-show rate and the gap from the desired no-show rate).
Goal Statement: Provide a SMART goal statement (only SMART goals will be accepted here) addressing the gap in quality. Represent this graphically as well as verbally. Define SMART goals (cited) before you begin.
Root Cause Analysis: Break down the data further and show significant drivers or factors contributing to the problem. Provide a logical connection between the observed data (effect) and cause(s), ideally identifying a root cause to be addressed through countermeasures. Include a Fishbone and 5 Whys related to highest impact cause.
Countermeasures: List identified problem causes and potential improvement interventions to be tested.
Effect Confirmation: Measure and show results of countermeasure testing in reducing the targeted effect of gap in quality. You will need to extrapolate expected results based on data given.
Follow-Up Actions: For example, you may list plans to adopt, adjust, or abandon tested countermeasures. This can include standardization, communication, change management, sustainment/control, and diffusion needs.
Note: Incorporate all relevant graphs into the section where they apply. No appendices should be required.
