You will formulate and communicate a theory-based approach to your health program. The approach must be grounded in existing knowledge about health promotion theory and known effective interventions, account for community priorities, and include clear and measurable goals and objectives. Use as wide a variety of resources as you can find to provide a complete explanation of your intended approach and justification.
The purpose of this assignment is to demonstrate your ability to justify the approach you intend to take to your program and its likelihood of achieving a positive health effect.

Literature Review of potential effective solutions (5pts)
Summarize the known effective interventions for your health issue in your program’s target community. Provide at least 2 examples of health behavior theories that help explain why these interventions work. Provide a summary of potential solutions that have not been shown to be effective and should be avoided. Utilize evidence from both the peer reviewed literature, public health institutions, and resources that reflect community stakeholder views.
Logic Model of Change (Effect Theory) (5pts)
Construct a visual conceptual model of your theory of change and clearly identify your program’s health outcomes of interest, the intermediate behavioral and environmental change outcomes, and your programmatic objectives. Provide a written annotation to explain and justify its contents; objectives should clearly relate to Healthy People 2030 goals.
Note: Your effect theory model should NOT represent your overall organizational plan -- just the relationship between your interventions, intermediate outcomes, & health outcomes.
Program components (10pts)
Select a specific intervention approach that you will implement to address this issue in your target community and describe it in detail, including any theory on which it is based, and its components, scope, and sequence of activities. Justify how these activities will help create? change in your intermediate outcomes.
Peer Review (5pts): Provide a formal peer review of one classmate’s needs assessment using a? template provided by the instructor. I will assign you a peer to swap with and the template.
