A 26 year old woman in your facility went to the Operating Room to have an appendectomy. Postoperatively, the patient became septic and required further surgical intervention. Upon returning to the OR, it was discovered that not only had her appendix not been removed, but it had subsequently ruptured, and her uterus, ovaries and fallopian tubes had been removed in the first procedure.
