Complete and submit the worksheet attached below. Please remember to upload only one file with both pages of the worksheet. You may (1) spread them out and take a single picture of them, (2) take individual pictures and combine them in a collage app, (3) take individual pictures and paste them into a single word doc, or (4) using an iPhone, use the scanning function in the notes app to scan each image and save as a single pdf. Acceptable file types are: jpeg, jpg, doc, pdf, png.
