Part I: 

 

Health education programs are produced and implemented based on health issues impacting a particular population. You will choose a health issue/health problem (ex: obesity, heart disease, diabetes, STI's, etc. ) that you feel impacts a particular population (you will not discuss the population in draft report #1) and discuss that health topic and problem. Your report should include: 

 

  • Introduction & statement of health problem
    • (define the topic and problem) 
  • Discuss the statistics of the health problem (ex: it impacts 50% of individuals in the United States) and how often the health problem occurs
  • Manifestations of the health problem, signs, & symptoms
    • How does the health problem begin, the signs, and symptoms of the health problem/how does it impact an individual or population

 

Part II: Target Population 

Now that you have successfully stated your health problem, it is time that you research your target population. Your target population is who your program aims to impact the most and who are most impacted by your health problem. Examples are college students, African Americans, Women, Hispanic Men/Women, etc. 

  • Introduce Target Population & Demographic Information
    • Population rates in specific area and United States 
  • Risk factors for health problem in target population
  • Why is your health problem impacting your target population? 

 

Part III: Program Mission Statement & Goals – Due March 8th

In this section of your program plan, you are now in full preparation planning your program. Refer to chapter 6 in your textbook to guide you as you plan your program.

  • Name your program (think of something catchy, yet directly relates to your health program & target population)
  • Create a mission statement (see pg. 135, table 6.1 for examples).
    • “The mission of “program title” is to provide….
    • “The mission of “program title” is aimed…..
    • “The mission of “program title” promotes….

 

  • Create the goal or goals of the program (see pg.136, Box 6.2 for examples). Make your program goals specifically align with your program and target population.

 

  • Create a program description answering the questions below. Your program description should answer the following questions in 4 to 6 sentences.
    • What is the significance of the program?
    • Why is the program important?
    • How will the program impact your target population? What will the learn?
    • What type of intervention strategy will you use? (See Chapter 8, pg.193-215)

 

Part IV: Program Recruitment- Due April 5th

 

Program recruitment will be key in the sustainability of your program. Please answer the questions below in at least 4 to 6 sentences.

  • How will you market your program to potential program participants?
  • How will your recruit participants?
  • What is the criteria for individuals to participate in your program?

Potential program participants will be recruited from the Martin Luther King Jr Health Center or they may be referred from other Harris Health Centers as they see a need for certain women to participate based on risk factors. Participants will be recruited and enrolled based on identifiable risk factors as noted in their Harris Health electronic medical record from previous health center visits. The goal is to enroll 120 women in the SWH program. Program inclusion Criteria includes:

  • African-American woman (required)
  • Between 23 and 45 years of age (required)
  • Elevated Blood Pressure Level/Hypertensive
  • Elevated Low-Density Lipoprotein (LDL) level
  • Elevated Triglyceride level
  • BMI over 30
  • Family history of heart disease
  • History of preeclampsia during pregnancy

 

Part V: Program Timeline- Due April 19th

 

Your program timeline is important in the impact of your program. Participants attendance is critical in the impact of your program and creating a sustainable behavior change and increase knowledge in your target population. See table below to get you started in completing your program timeline. Your program should be a minimum of 4 months. It can start at any time of the year.

Part VI: Program Impact- Due April 25th (with the submission of final paper)

The final portion of the program should focus on the impact of the program.  What type of impact will your program have on the program participants in the four months of the program? Will they gain knowledge? What type of health behavior change will participants make after participating in the program?

Formatting

Please save your document as LastName_FirstName_Final Health Program Plan. The final draft (due April 27th) should include a title page ) that includes title, name, date, course, and semester. All documents should be written in Times New Roman, 12-point font, double spaced. Also, be sure to include your references in-text and on your reference page (Parts III- VI do not need a separate reference page).
