• Help the patient to provide a driver's license or birth certificate as proof of age.

• Help is providing the required documentation to identify citizenship

• Help gathering records of income as an income source. 

• Any home, car or other assets document proof

• If any physical disability is present then help your patient to get a disability certificate by communicating with a specified physician or doctor mentioned by Medicaid. 

• .Prepare set of proof of residence for your patient

• Copies of insurance already clamming, i.e white, red, blue Medicare card or other cards related to insurance

• Any other documentation that may vary from state to state.

You have to collaborate with physicians or doctors for providing a medical certificate. disability certificate in case any disability is present in the patients. Other nurses and Medicaid information desk.
