Cases 8.2

Submit case 8.2 first designing the survey question for each of the 5 questions in the case ( for an example of survey questions review Table 8.1 and table 8.6) (each question is worth 2 points) and then create an on-line survey instrument (no need to actually run the survey or create it online just what it would look like)that incorporates each of these five questions and other important pieces of information (see table 8.7 for best practices when creating the on-line survey)(survey is worth 5 points).
