The goal of your project is to identify barriers in the workplace to understanding vulnerable populations and affecting the disparity in promoting health care delivery. ThisProject presents the opportunity to take course concepts and knowledge to your workplace to affect change and create new awareness to the realities of health disparitiesAnd barriers to health care that challenge wellness interventions.Identify a barrier to decreasing a health disparity in a vulnerable population.Evaluate your workplace for the presence of barriers in understanding vulnerability.Use the action plan approved by your facilitator to complete this project.Develop your action plan with the objective of increasing awareness of vulnerability based on culture, illness or disease, or a goal of Healthy People 2020.Design the materials for your action plan for your workplace. You can teach a class, create a poster for display in your unit, design a brochure for staff, or use another creative approach. Include the following elements:An audio or visual aid as a tool to your action planThree integrated course conceptsOther applicable materials such as the teaching aid, digital photos, a copy of the brochure, and an outline of the teaching plan, as appropriateWrite a 700- to 1,500-word report describing experience in the workplace.
