A nurse is assigned to a recently admitted 70-year-old client with depression. The client’s son brought her to the extended-care facility 3 days ago. His job has transferred him 500 miles away, and he and his family will be relocating closer to his work. The client lived with them for the past 4 years after she suffered a stroke that left her legally blind. She was able to care for herself with the help of a home health attendant (HHA). Her son did not feel that it would be safe to leave her in the home alone because the HHA is only there 6 hours, 4 days a week. During assessment, the client tells the nurse, “You must know that I cannot see very well now. I find it very difficult to complete important activities. Most nurses just leave me to do things by myself. They don’t seem to care very much. They just hurry about their business.”
