Please go to the Google Forms training and help center (Links to an external site.) shown in Figure 1 below. You can also use the Google Search engine to search for Google Forms Help and you should see several links for help with Google Forms.

On the Get started with Forms page, be sure to read Sections 1-5 in their entirety as shown in Figure 2 below:

After reviewing the Google Forms tutorials, let us assume that you are organizing a field trip for your classmates to National Landing which is what the area of Arlington, VA will be referred to where Amazon plans to operate H2Q. NOVA has a Field Trip form that needs to be updated and made available as a Google Form. You can see an example of the old form in the following image:

As you create the new form, please make the following changes:

• Replace SSN with Student ID
• Replace Telephone Nome with Cell Phone
• Replace Work with Emergency Phone
• Add a field where students can check a radio button (Yes) or (No) under the heading of Transportation Needed:
• You do not have to add the wavy NVCC logo.  Choose a color scheme or theme that is appropriate.

Once your form is ready for your instructor to grade, send the  link to the form via email to your instructor.
