Discuss how you explained the initial model to your client and how you formulated the treatment plan. Then discuss how you used each component of the chosen therapy to treat your client's condition. For instance, if you use TF-CBT, discuss how you implemented each component and how you used fidelity checks to make sure you were delivering the therapy in an appropriate manner. If you used a more general form of CBT to treat an adult, explain how you described the cognitive model and how you helped the client identify automatic thoughts, feelings, and behaviors that were not useful. 
Then, if you used CBT, describe how you helped your client identify distorted thinking and intermediate and core beliefs, how you helped them reframe their automatic thoughts and core beliefs, and how and if your client's feeling intensity decreased. You can audiotape a 10- to 15-minute clip of a session with a client (with their permission, of course) and transcribe it as a way to gain a fidelity check but this is not a requirement.  
Then, discuss how you used techniques (e.g., problem-solving, imagery, homework, or other behavioral techniques) to help the client change their behavior. Discuss your therapeutic outcomes with the client, including any practice evaluation you did and whether or not you feel adequate progress was made with your client. Identify any barriers that occurred during the therapy process and how you addressed these barriers. 
Discuss any adaptations (e.g., cultural) that you had to make to the therapy process and your rationale for the adaptations. Finally, discuss your planned follow-up/transfer or termination plan with the client and discuss how this case has impacted you as a therapist and individual, and how you will use this information to inform future clinical work.
