Review Chapter 7 (Integrated System Hazard Analysis) and Chapter 8 (Safety Analysis: Hazard Analysis Tasks – sections 8.1 – 8.9) of the FAA System Safety Handbook found in the Readings & Resources section of Module 3.
Select one of the diagrams depicted in Fig. 7-1, 7-2, or 7-3, and perform a hazard and risk analysis for your chosen system.
You will need to estimate the mishap risk index associated to each hazard.
Please use the FAA hazard analysis templates (Excel worksheets). Your analysis should include the following worksheets: Preliminary Hazard List, System/Subsystem Hazard Analysis and Functional Hazard Analysis.
