HIM 400 Final Project Scenario ABC Medical Center Scenario and Data Set Problem: ABC Medical Center is considering adding group visits for patients to learn and share information about medical issues. The diagnoses that are being considered are adult-onset diabetes (E11.9), hypertension (I10), and congestive heart failure (I50.4). ABC Medical has investigated data for patient wellness programs offered by other organizations and has determined that females above the age of 50 and males above the age of 68 are more likely to participate in such groups. In order to justify the expense of a patient wellness program, ABC Medical needs to determine if there is a sufficient population of patients with the indicated diagnoses and if that population falls within the age parameters. Organizational Information: Multiple departments have come together to provide the data below. Some departments use paper records, and other departments use electronic methods. Currently, patients bring in paperwork that they fill out at home. Then, the paperwork is transcribed by the hospital into the EHR. Data entered must be in the following forms: ?
