Logic Model (Week 6)

For this assignment: 

1) Create at least a 1-page logic model based on the samples provided or use a model design of your own.  Make sure your model  contains the following columns:  Goal, objective(s), Inputs, Activities, Outputs, Outcomes (both Short and Med/Long term), and Impacts

2) Provide the narrative (approximately 3-5 pages but the page count may be more) for the logic model using the aforementioned elements as headers in your paper (see examples in the supplemental folder)

3) Include ELCC standards citations where you are applying the standards.  Some students are citing the standard like an in-text citation (ELCC 1.4) while others have created a new header and addressed their application of the ELCC standards in a separate paragraph.  I am good with either method.  I have included both the Building level and District level ELCCs for those who are implementing their evaluations.  Although this course focuses on Building level standards please feel free to use district level standards if you are a district level administrator and if they fit better into your evaluation proposal.
