Community-based program design often utilizes a range of tools and models which are meant to enhance the efficacy and outcomes of the program's design.

Your proposal is a community-based program that consists of any population and located in any part of the state of (whatever state). You have a total of $250.000 over a 3-year time period.

Part II:           Need Statement (At least two-pages identifying and explain the problems and a clear description with statistics).    

Part III:          Program Content and Structure (A good description of the structure of the program)

Part IV:          Project Goals objectives: (Include measureable activities or benchmarks for each one) 

 

Part V:           Project Budget (3 year budget description, include any in-kind and fringe benefits) (Fringe benefits are 25% of salary)

 

Part VI:          Timeframes (3 year plan- month by month planning)
