a nurse is assigned to a recently admitted 70-year-old client with depression. the client's son brought her to the extended-care facility 3 days ago. his job has transferred him 500 miles away, and he and his family will be relocating closer to his work. the client lived with them for the past 4 years after she suffered a stroke that left her legally blind. she was able to care for herself with the help of a home health attendant (hha). her son did not feel that it would be safe to leave her in the home alone because the hha is only there 6 hours, 4 days a week. during the assessment, the client tells the nurse, "you must know that i cannot see very well now. i find it very difficult to complete important activities. most nurses just leave me to do things by myself. they don't seem to care very much. they just hurry about their business."

a. The nurse should originally respond to the client by asking an open-ended question, such as "tell me more about the situation that you're describing." The nurse can begin to build a trusting relationship with the client by exploring their feelings about a past-situation. Importantly, the nurse must rebuild the potential loss of trust that the client may feel about the healthcare team. Additionally, the nurse should ask more about the client's visual impairment, such as "what types of activities do you have difficulty performing", and "how involved do you like to be in your daily activities?" Again, by gathering more information from the client, the nurse shows active engagement with the patient in developing a patient-centered plan of care.

b. Non-verbal communication is a communication that does not use words. They said that it is the oldest form of communication. They can be intentional or non-intentional as you show it to other people. Non-verbal communication is usually touch, gaze or eye contact, volume, posture, dress, smell, body movements, gestures and facial expressions.

Responding to non-verbal communication mean you acknowledge of the intended meaning o fthe non-verbal cues. For an example, babies or childres they use non-verbal cues, but most of the time espacially the mothers, they knew what their babies mean, like milk, sleep or wet nappies.
