1. You are a new health department nurse who has been tasked with working with the Cherry Hill Neighborhood in Baltimore. Please write about what you would do to begin working with this community to improve health outcomes. What role does epidemiology play in your work? 2. Please write about the impact of fragmented care on health. What does fragmented care look like in a clinical setting. Please identify two evidence-based strategies you as a   CNL could use to reduce fragmentation and improve health outcomes in your specific area of interest. 3. You are the community/ public health nurse working with a group of pre-teens in the Sandtown-Winchester neighborhood middle school. You recognize that in order to reduce the rates obesity for this group of girls you will need to consider interventions at all levels of the socio-ecological model. You will be working collaboratively with the school nurse at the middle school. Please propose an intervention ( Minnesota Wheel ) that would be appropriate at each level of the socio-ecological model. (you can pick a different intervention as each level).
